Registered Dietitian: Helps you make food choices specific to your needs to support your treatment and overall health
Pharmacist: Reviews your medications with you and helps you manage your daily medication schedule
Social Worker: Provides you, and your family or loved ones with on going support, counseling, education. Social workers can help refer you to other resources to maintain your wellbeing
Nurse Practitioner: Helps to manage your kidney illness and treatment. The Nurse practitioner will also work along with the team to focus your overall plan of care.
Registered Nurse: The registered nurse will assess you upon your clinic visit. The registered nurse works alongside the doctor to educate about your illness and treatments.
