COSS is a mobile crisis intervention and outreach service for seniors that provides short-term response 365 days a year, 7 days a week, 9am to 5pm, through in-person support. The team provides interventions including crisis counselling, short-term intensive case management, support around harm reduction, concurrent disorder, mental health and addictions, health assessment and care as needed by a nurse practitioner and referral to long term primary care
Intake is done by phone, no referral form required.
