Much is known of health outcomes for Indigenous and queer populations, but what are local health providers in urban areas doing to support their two-spirit and queer Indigenous community members?

As the world population continues to increase, so does the number of older people. According to the United Nations' World Population Aging report, one in five people will be over 65 by the year 2050, providing greater visibility of the diversity among older people, particularly with respect to sexual orientation and gender identity. For instance, more than 39 million people in the U.S. are age 65 years or older including 2.4 million people who identify as lesbian, gay bisexual, transgender, queer, intersex, asexual and other (LGBTQIA+). Since older people represent a growing demographic group, new and important challenges around successful and dignified aging arise for those who belong to gender and or sexual minority groups.

To provide inclusive, affirming services, and welcoming health care to LGBT2SQ seniors, healthcare and social service providers should appreciate that not all seniors are heterosexual and cisgender. As they approach their twilight years, LGBT2SQ seniors are confronted with uncertainty about how to navigate health care and social services. Much of this uncertainty is because of historic stigmatization and discrimination towards LGBT2SQ communities, which has defined the lived experiences of many LGBT2SQ seniors.

Historically, people with minority sexual and gender identities (e.g., gay, lesbian, bisexual, transgender) have been pathologized by mental and medical health practitioners. The potential for pathologization of asexuality is particularly salient considering a lack of sexual desire or interest has been studied in relationship to depression, antidepressant medication, and hypothyroidism. To explore this potential pathologization, asexual individuals were asked about their interactions with mental health and medical practitioners.

This narrative review describes best practices for delivering culturally sensitive care to transgender patients within the context of chiropractic offices.

Providers have the opportunity to intervene and positively impact patient experiences through gender-affirming care, but they first require an adequate knowledge base and understanding of the importance of sensitive and inclusive care. Seemingly small interventions, such as listing ones own pronouns, using genderneutral language, validating and affirming patients, and utilizing appropriate mental and physical health screenings, can lead to significant impacts on the patient experience, health outcomes, and quality of life. This article will discuss some of the most common disparities and obstacles faced by transgender patients and will argue the paramount role of the provider in establishing gender-affirming care and some high-impact avenues which the provider, regardless of specialty, may pursue when caring for these patients.

The clinician's use of non-stigmatizing language reflects the patient's full identity, and acknowledges the patient's capacity to change and grow, by avoiding words or phrases that could be perceived by the patient as dehumanizing, that perpetuate stigma, and that marginalize rather than support.

The clinician's inquiry and use of the patient's preferred name, title, and gender pronoun promotes a welcoming environment—especially for a patient whose preferred pronoun or name does not conform to historically conventional societal norms.

Whether the clinician is interviewing the older adult patient in an office, hospital, or nursing home, adjusting the physical environment and the interviewing approach to put the patient at ease will result in better communication and more effective medical treatment.

LGBTQIA+ patients who experience high rates of health care disparities and bias are less likely to express their health care concerns. The successful interview starts with asking the patient how they would like to be addressed, including their preferred pronouns. A supportive, nonjudgmental approach allows the clinician to stablish rapport with LGBTQIA+ patients and explore all facets of their health and well-being, including sexual orientation and gender identity (SOGI).

An introduction to the history of Two- Spirit people in American Indian and Alaska Native communities (AI/AN), which will include an overview of the displacement of this population. Topics such as historical and current trauma, past and current discrimination, and the impact on Two-Spirit roles in tribal communities will be presented and discussed.
