The Kidney Care Clinic

Welcome to the Kidney Care Clinic

When arriving at the Kidney Care Clinic, you will be introduced to your doctor and kidney care team. The healthcare providers assisting you will include the registered dietitian, pharmacist, social worker, and nurse practitioner.

Your first visit will be about two hours. Future visits will be much shorter and more focused on your personal needs and concerns.

Before your clinic visit, make sure you take your scheduled prescription medication.

In addition, remember to bring the following:

• Your Ontario Health Card
• All current prescription medications in original packages and bottles, over the counter medication, vitamins, herbal and traditional supplements
• Blood pressure, weight, and blood sugar log book if applicable
