Your phone appointment

How do phone appointments work?

We will send you a letter with information about your appointment. Read it carefully. If you have any questions or need to rebook your appointment, call us at (416) 867-3679.

Note: If we call you, our number may show up as “No Caller ID” or “Unknown” on your phone. Please change the setting on your phone to accept calls from private numbers.

 

Before your first appointment

Upload the data from your glucose monitor, insulin pump or blood sugar meter. You may also want to have these things ready for the day of your appointment:

  • A quiet, private place where you can easily talk to us over the phone
  • A list of questions you would like to ask during your appointment
  • Your blood sugar meter, glucose sensor and/or insulin pump
  • Your blood sugar logbook
  • All your medicines
  • Your pharmacy phone and fax number

On your appointment day

We will do our best to call you at the time stated in your letter. If we are helping another patient with an urgent medical problem, we may be a little late. Please wait for us, we will be with you as soon as we can.

After your appointment

If your healthcare team member is booking you for another appointment, you will receive a letter with the appointment information. It will also have information on any medical test or bloodwork we would like you to have done.
